Out Lunch Consent Form

MS 915 Families:

We are pleased to announce that all students will have to opportunity to enjoy ” out lunch” this year. Out lunch requires a great deal of responsibility and independence on students’ parts. We encourage you to carefully review the guidelines listed below, with your children before granting them permission. Only students with parental permission are allowed to participate in ” out lunch.”

“Out lunch” Logistics

Where?

There are parameters! Students may enjoy lunch in speciﬁed areas of Metro Tech Park, sitting on the park benches eating their own lunch or purchasing lunch from one of the local establishments listed below. Students are not permitted to go beyond the boundaries highlighted on the map below:

When?

Lunch is Period 6. All students go directly to the cafeteria after period 5. Students who are going out will sit at the tables to the right of the door that leads to the main lobby and when dismissed, walk to the computer station in front of the security desk at Johnson Street. They will swipe out with their CAASS ID card. Ms. Spitzer, the guidance counselor will be there to ensure students are swiping their cards. No student may go out to lunch if they do not have an ID. (STUDENTS WILL BE ABLE TO ATTEND OUT LUNCH WITHOUT THEIR ID UNTIL ID’S ARE DISTRIBUTED.)

Guidelines

Out lunch is a privilege that we take very seriously at MS 915. Staff is not present to supervise students during out lunch. Students must demonstrate the ability to independently manage their time and behavior in order to remain eligible for out lunch. Students and families should familiarize themselves with the following guidelines:

Students must have the attached signed permission slip on ﬁle at MS 915 in order to participate in out lunch.

Students must uphold the behavioral expectations of MS 915, both inside the school building and out, in order to remain eligible to attend out lunch, regardless of the signed permission slip.

If students return to school late from out lunch, their pass will be taken away for the following day. Students who demonstrate a pattern of lateness will lose out lunch privileges indeﬁnitely.

Students must clean up after themselves in the Metro Tech Park and inside the various establishments.
No student may buy food for others when going out.

Please review this information carefully with your child at home. If you would like to give your child permission to attend out lunch, please complete the form below. A copy of your entry will be sent to the email address provided.

PLEASE NOTE THAT THIS FORM MUST BE SIGNED IN ORDER FOR YOUR CHILD TO GO OUT FOR LUNCH. THIS FORM IS NECESSARY AS IT IS A CONTRACT BETWEEN THE SCHOOL AND THE STUDENTS, WHO AGREE TO ABIDE BY THE RULES OF THE " OUT LUNCH" PROGRAM. THANK YOU FOR YOUR COOPERATION.
